New Patient Registration

We normally only accept new patients who live in our practice area of Earls Barton and the surrounding villages but we are flexible in recognition of the Government proposals to remove practice boundaries. If you wish to apply to become a registered patient you can collect a registration form from reception or download one from the above “Registering Online” tab.

The practice will consider your application and contact you to confirm acceptance. To complete your registration you will need to provide proof of identity and evidence of entitlement to NHS services.

When you register, we will offer you the opportunity to make an appointment to see the health care assistant for a new patient health check. This will ensure that our records are accurate and all routine checks to promote your health and wellbeing are up-to-date.


All patients registerig with Earls Barton Medical Centre will automatically be allocated a 'named accountable GP (usual GP)'

You will be notififed of your named GP once your registration hase been processed. Please ensure you contact the surgery within 7 days of submitting your application to esnure your application has been processed sucessfully.
