PRIMARY CARE NETWORKING
From July 2019 all patients in England will be covered by a Primary Care Network (PCN) which is the most significant reform to general practice in a generation.
Every practice has been encouraged to form local primary care network teams typically serving communities of around 30,000 to 50,000 patients. Small enough to provide the personal care valued by both patients and GP’s but large enough to have an impact and economies of scale through collaboration between practices and the local health and social care system
Primary care networks build on the current core services to enable greater provision of proactive, personalised, coordinated and more integrated health and social care.
PCN’s should help to integrate primary care with secondary care and community services to help bridge the gap between general practice and integrated care
